IVF vs ICSI: Understanding the Difference & When Micro-Injection is Needed

In conventional IVF, thousands of washed sperm are placed into a petri dish alongside retrieved eggs, and one sperm naturally penetrates each egg's outer layer.

In Intracytoplasmic Sperm Injection (ICSI), an embryologist uses high-magnification micromanipulators to select a single, morphologically normal sperm and injects it directly into the cytoplasm of a mature egg.

ICSI revolutionised reproductive medicine because it overcomes severe male-factor infertility barriers — including very low sperm counts (oligozoospermia), poor sperm motility (asthenozoospermia), or high abnormal forms (teratozoospermia).

ICSI is also utilized when sperm has been surgically retrieved from the epididymis or testicle (PESA/TESA), in cases of previous poor fertilization with conventional IVF, or when pre-implantation genetic testing (PGT) is planned.

At Srishti Fertility Hospital in Mysuru, our embryology team applies ICSI selectively based on clinical indication rather than performing it indiscriminately, ensuring ethical, evidence-based care.
